SAFETY ALERT
On Monday April 4, 2005, a serious issue with a safety
lanyard was found by one of our customers. While
using a Safewaze 3550-0241 lanyard, one leg of the
lanyard pulled apart. This was the result of a
major error in the manufacture of this lanyard and the
failure to identify this problem during multiple
inspections of the product.
On the nylon webbing was a section that was sheared
and subsequently taped during the spooling process
before it was shipped to Safewaze. Apparently
this is a common practice by the manufacturers of web
products when spooling the product prior to shipping.
The tape should be a much different color than the
webbing in order to avoid using this section of the
product. For this reason, we must "IMMEDIATELY
INSPECT ALL Fall Protection Web Products for any
use of adhesive tape. The tape is to be removed
immediately in order to determine if A) there is a
seam in the web beneath the tape, and B) to determine
if there is any evidence of damage or defects beneath
the tape."
Ensco Supply will immediately inspect all inventory
for tape on any of its web products. If any such
product is found, we will remove the product from
inventory, notify Richard Crifasi via cell phone or
e-mail, and hold for inspection by a qualified
individual.
We must also communicate this potential condition to
all our customers advising them of this potential
condition, so that they can arrange for an immediate
inspection of all their webbed fall protection
products. We are to offer this inspection at no
cost to our customers and advise them of the danger in
using tape over web product for inspection or
identification purposes. Safewaze will provide
an alternative method for labeling fall protection
products so we do not provide any opportunity for the
use of sheared or damaged product from this day
forward.
